a.) Review and return completed "BB Wholesale Terms & Conditions" doc b.) Complete gray shaded areas and retailer signature portion of "BB Order Form" c.) PRINT and FAX completed "Wholesale T&C" to: 828.414.4534 or **SCAN and EMAIL to: wholesale@burkleighbaby.com (must have signature on form) d.) **EMAIL completed "BB Order Form" as an attachment to: wholesale@burkleighbaby.com (type your name in the retailer signature section) **An actual signature is required on the "Wholesale T&C" document. Once you have completed that form, we will accept all "BB Order Forms" with the retailer name "typed" on the line requesting a retailer signature. |
